Shift: 8 Hour Days, M-F (flexibility with 4-10s possible)

Hours per Week: 38-40

Employment Type: Permanent placement

License/Certs: Minnesota Physical Therapist license, Basic Life Support (BLS) within 1 year

Experience: Minimum 1 year in healthcare setting

Annual Base Salary: $80,000 to $115,000 (dependent on experience)

Sign-On Bonus: Up to $10,000 with a 2-year commitment

What You'll Do

• Provide outpatient and inpatient physical therapy services across various settings, including rare SNF/Home Health cases
• Document patient progress and treatment plans using Epic EMR system
• Collaborate with healthcare team to develop individualized care plans
• Assist in patient assessments, therapy interventions, and discharge planning
• Support community health initiatives by providing therapy to local school children as needed
• Participate in rotation for weekend shifts (every 6-7 weeks) and be available for rare holiday coverage

What You Need

Required:

• Valid Minnesota Physical Therapist license
• BLS Certification within 1 year
• Minimum 1 year of experience in a healthcare setting
• Proficiency with electronic medical records (preferably Epic)

Preferred:

• Experience in outpatient and inpatient rehab services
• Ability to work flexible schedules
• Strong communication skills and team-oriented mindset
