**Job Title:** Home Health Aide (HHA)

**Job Summary:**

As a Home Health Aide (HHA) with SYNERGY HomeCare of Woodbridge, you will play a vital role in delivering high-quality care to patients in their own homes. Our team is dedicated to providing compassionate and personalized support to individuals in need, and we're looking for a compassionate and skilled HHA to join our family.

**Responsibilities:**

* Provide personal care services such as bathing, dressing, and grooming to patients in their homes
* Assist patients with daily living activities, including meal preparation, light housekeeping, and medication reminders
* Monitor and report changes in patient condition to healthcare professionals
* Maintain accurate and detailed records of patient care and services provided
* Collaborate with healthcare teams to develop and implement patient care plans
* Demonstrate empathy and understanding towards patients and their families, promoting a positive and supportive environment

**Requirements:**

* High school diploma or equivalent required
* Current certification as a Home Health Aide (HHA) in the state of New Jersey
* Ability to work independently and as part of a team
* Excellent communication and interpersonal skills
* Ability to lift, transfer, and move patients safely and comfortably
* Valid driver's license (if applicable)
