Position Overview:Under direction and supervision of the Behavioral Health Manager, the Behavioral Health Community Health worker is responsible for providing Comprehensive Community Support Services (CCSS) for adults with significant behavioral health disorders (Serious Mental Illness and/or Substance Use Disorder (SUD) and their families AND for children or adolesce experiencing Serious Emotional/Neurobiological/Behavioral Disorders and their families, to improve quality of life and maintain the highest possible ability to function independently within the community.What You Will Do:Support the Behavioral Health department and other departments within ECFH with effective communication to include but not limited to answering phone and scheduling appointments.Assist with coordination of behavioral health referrals, warm hand-offs and maintain an active list of community resources.Assist the behavioral health providers with primary screens (i.e., risk assessments, PHQ-9, GAD-7, ACE. PCL-5, social determinants of health) to identify client needs and to assist with the data entry such as the Treat First process.Support the Behavioral Health Department to include the Behavioral Health providers with support services such as but not limited to: Behavioral Health intakes and screenings, comprehensive community support services (CCSS), screening brief intervention and referral for treatment (SBIRT), group facilitation, and support services at the colleges and School Based Health Centers (SHBCs). This position is allocated 90% to direct care coordination and support services, 10% to direct patient services such as SBIRT/CCSS.Facilitate patient access to community resources, including locating recreational activities, housing, food, clothing, school programs, vocational opportunities, or services, and assists clients to develop natural resources and make contact social support networks.Teach and assist patient(s) with coping skills, daily living skills, skills training, resource utilization training, goal attainment planning, socialization skills, etc.Assist in teaching symptom monitoring and illness self-management skills including symptom management, relapse prevention, medication knowledge including medication side effects, and motivational/skill development in taking medications as prescribed.Assess support and intervenes in crisis situations, including the development and use of crisis plans that recognize the early stages of crisis/relapses, and use natural /community supports as alternative to hospitalization or emergency room visits.Minimum Qualifications:High school diploma or GED equivalentCardiopulmonary resuscitation (CPR) or Basic Life Support (BLS)Exceptional written and verbal communication skills, with the ability to convey information clearly and professionally.Proficiency in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint) and other standard office technologies.Ability to perform duties and responsibilities with dignity, calm, accuracy, and confidentiality in all contacts with employees, managers, and the public.High level of emotional intelligence, strong listening skills, and the ability to respond thoughtfully and appropriately in varying situations.Professionalism and discretion in handling sensitive and confidential informationPreferred Qualifications:Education or Experience in Psychology, Sociology, Social Work, Criminal Justice, Family Studies, Special Education and Human Service or a related field.Comprehensive Community Support Specialist (CCSS) or ability to obtain certification within 90-120 days of employment.SBIRT-Twelve (12) hours of motivational interview training or ability to obtain certification within 90-120 days of employment.Exposures:Airborne PathogensBlood borne Pathogens-Category IChemicalsElectrical current; machine vibration; moving parts.Muscular and muscular strainNoiseOdors, fumes, toxicants, explosive materials, and chemicalsTemperatureVision and hearing strainWhat We Offer:401(k) Retirement Plan7 Paid HolidaysPaid Sick TimeComprehensive Medical, Dental, and Vision Insurance100% Employer-Paid Basic Life InsuranceVoluntary Employee Supplemental BenefitsEmployee Assistance Program (EAP)Education ReimbursementFlexible Spending Account (FSA)Who We Are:As a Federally Qualified Health Center (FQHC) and 501(c)(3) nonprofit, El Centro Family Health is dedicated to providing affordable, accessible, and high-quality healthcare to the people of Northern New Mexico.
