Job Summary

The GPD Program Manager leads VTC’s transitional housing program and supervises the case management and intake team. The Manager is accountable for daily operations, staff performance, participant service coordination, documentation, and assigned grant administration. The role focuses on helping Veterans move into permanent housing and connect with the resources needed to maintain stability.

The Manager provides administrative and case management supervision, direct participant support, and coordination with the VA GPD liaison. Clinical assessment, treatment, and clinical supervision are performed only by personnel with the credentials and authorization required for those services.

Essential Functions

Staff leadership and daily operations

• Supervise four Case Managers and one Intake Coordinator; assign caseloads, monitor work quality, provide coaching, and complete performance evaluations under VTC policy.
• Lead weekly staff meetings to review housing barriers, participant needs, service coordination, and follow-through on assigned tasks.
• Arrange staff coverage and provide case management backup during absences within the Manager’s competence and assigned responsibilities.
• Coordinate orientation and continuing staff training in documentation, professional boundaries, motivational interviewing, trauma-informed care, de-escalation, and crisis response.

Intake and housing planning

• Work with the Intake Coordinator and VA GPD liaison to document eligibility and required VA authorization; coordinate timely admissions consistent with the funded model and applicable access requirements.
• Oversee individualized service plans that identify each Veteran’s strengths, needs, housing barriers, goals, planned services, and referrals. Engage Veterans in developing and updating their plans.
• Begin permanent housing and discharge planning at admission; review progress with staff and coordinate with the VA liaison team at least weekly.

Participant services and safety

• Coordinate access to healthcare, mental health and substance use treatment, benefits, employment assistance, life skills, transportation, food, and community supports. Confirm that referrals result in appropriate follow-up.
• Work with VA teams, housing providers, local coordinated entry partners, the Veterans Service Office, Social Security Administration, and other community agencies to resolve barriers to housing.
• Provide de-escalation and crisis response within training and scope; activate emergency services and qualified clinical support when needed. Maintain staff readiness to follow suicide prevention and emergency procedures.
• Ensure sentinel events are reported to the VA GPD liaison within 24 hours; follow applicable reporting requirements for other critical incidents and document follow-up.
• Ensure housing units are inspected at least weekly under VTC policy; coordinate repairs and safety follow-up with facilities staff. Support VA inspections, emergency planning, and required corrective actions.
• Oversee participant orientation, rights, grievances, and discharge procedures, including documented referrals and continuity of services.

Documentation and program quality

• Review and approve weekly case notes for all participants and conduct monthly participant file reviews. Require timely corrections and clear documentation of services, progress, and discharge outcomes.
• Ensure service outcomes are verified with participants and documented each calendar quarter. Maintain accurate enrollment, attendance, absence, and discharge records.
• Protect participant information, maintain appropriate releases, and follow applicable confidentiality requirements, including HIPAA and substance use record protections where applicable.
• Monitor occupancy, length of stay, permanent housing placements, negative exits, and applicable employment outcomes against the funded model’s targets. Report trends to leadership and implement improvement plans.
• Oversee accurate entry into VTC’s case management systems and HMIS where used or required by an applicable agreement; supply information needed for VA reporting.

Grant administration and fiscal coordination

• Implement applicable GPD requirements, the approved application, award terms, and VTC procedures; track deadlines and escalate compliance issues promptly.
• Prepare and reconcile monthly per diem billing with Finance using authorized admissions, eligible bed days, documented absences, and approved rates. Resolve discrepancies before authorized submission.
• Work with Finance on program spending, allowable costs, funding allocation, and supporting records for financial reports, rate requests, audits, and grant reviews.
• Assist leadership with renewals, proposed program changes, and required VA approvals. Perform other duties consistent with this position.

Qualifications and Employment Conditions

Required education and experience

• Master’s degree in social work or a related clinical field.
• Experience responding to crises and working with individuals who have complex housing, behavioral health, or social service needs.
• Demonstrated ability to supervise and coach staff, coordinate case management, and maintain accurate records.
• Ability to interpret grant requirements, prepare reports and procedures, communicate effectively with participants and partners, and use program data to identify problems and improve services.

Preferred qualifications

• Experience with VA GPD or comparable transitional housing programs, Veteran services, and grant reporting or per diem billing.
• Knowledge of substance use disorders and recovery principles; experience with motivational interviewing and trauma-informed care.
• Current, applicable substance use counseling certification or professional clinical license.
• Service in the United States Armed Forces.
• A degree alone does not authorize clinical practice or clinical supervision. Any assigned clinical duties require the applicable current license or credential and must remain within its scope.

Additional requirements

Complete background screening consistent with applicable law and position requirements. Maintain a valid driver’s license, an acceptable driving record, automobile insurance, and access to an automobile for assigned travel. Work independently, exercise sound judgment, maintain professional boundaries, and communicate respectfully with Veterans, colleagues, and community partners.

Work environment and physical demands

Work takes place in offices, residential settings, and community locations, with local travel and exposure to changing weather conditions. The role may involve contact with people in crisis and potential exposure to blood or body fluids; staff must follow VTC safety procedures and use appropriate protective measures.

Essential activities include communicating with participants and staff, using a computer, reviewing records, moving through residential settings, and traveling locally. Duties include lifting and moving items up to 30 pounds. Essential functions may be performed with or without reasonable accommodation.

Schedule and benefits

The regular schedule is Monday through Friday, 8:00 a.m. to 4:00 p.m., with occasional evening or weekend work as duties require. Paid hours, meal periods, and any after-hours coverage follow VTC policy and applicable law. Annual salary: $70,000–$80,000. Benefits include a 401(k) with employer matching; medical, dental, vision, and life insurance; and paid time off, subject to plan terms and eligibility.
