Case Manager

Benefits: Health insurance Paid time off Training & development Position Summary: The Clinical Case Manager is responsible for assisting the Clinical Director in coordinating treatment services including initial and ongoing coordination of care, advocacy, behavior intervention, aftercare, follow-up, and discharge planning. Case Management will work directly under the Clinical Director to ensure all policies and procedures regarding client care, discharge planning, and case management service needs. Essential Functions Conduct comprehensive assessments to identify clients' needs and develop individualized care plans. Coordinate and advocate for the delivery of a range of services to meet clients' substance abuse treatment goals. Collaborate with Clinical Director, healthcare team and external agencies to ensure continuity of care. Provide support and guidance to clients in accessing community resources and support services. Document all notes into EMR accurately and complete as it pertains to interaction with patients and the communication of their discharge plan and aftercare schedule. Facilitate communication and collaboration among the treatment team to ensure a cohesive and person-centered approach. Monitor and evaluate clients' progress toward treatment goals to assist with aftercare planning. Participate in team meetings to discuss client care and treatment plans. Provide crisis intervention and support as needed. Performs other duties as assigned by the supervisor, which are consistent with the position and in compliance with agency policies and procedures. Qualifications Bachelor’s degree in social work, human services, or a related field, preferred. Proven experience in case management, preferably in a mental health or substance abuse setting Strong organizational and communication skills Ability to collaborate with a multidisciplinary team and external agencies Knowledge of community resources and support services Commitment to promoting a culture of diversity, equity, and inclusion Previous experience in addiction treatment setting or with treatment of co-occurring mental health and substance use disorders preferred #J-18808-Ljbffr COBB OUTPATIENT DETOX LLC
