Responsibilities:

What You’ll Do:

• Deliver nursing care as outlined in each patient’s personalized care plan.
• Partner with RNs, therapists, and other clinicians to ensure top-quality care.
• Monitor and report patient conditions and changes with accuracy.
• Build meaningful relationships with patients and their families.
• Provide comfort, dignity, and hope with every visit.

Qualifications:

What You Bring:  

• Active LPN license in Pennsylvania.
• A passion for home health and patient-centered care.
• Strong communication and time management skills.
• The ability to work independently—with full team support behind you.
