PHARMACIST – HOME INFUSIONLocation: Syracuse, NYSalary: $124,800–$145,600Experience: 2+ Years Acute Care OR 1+ Year Home InfusionRequirement: Active New York Pharmacist LicenseBenefits: Begin Day 1POSITION OVERVIEWOur client is seeking a Pharmacist to provide clinical pharmacy services for patients receiving home infusion therapy.You’ll oversee medication therapy, IV compounding, patient monitoring, education, and coordination of pharmacy services while collaborating with physicians, nurses, and other healthcare professionals.WHAT YOU’LL DO• Evaluate patients and prescribed therapies for home infusion care. • Develop and maintain individualized pharmacy care plans. • Review labs, perform pharmacokinetic dosing, and make clinical recommendations. • Dispense medications, supplies, and infusion equipment. • Educate patients and caregivers on medications and infusion therapies. • Monitor patient progress, compliance, labs, and treatment outcomes. • Ensure compounding and clean-room activities meet USP <797> standards. • Coordinate refills, supplies, labs, compounding, and deliveries. • Supervise and support pharmacy technicians and other pharmacy personnel. • Participate in on-call responsibilities as required.WHAT YOU’LL BRING• Active New York Registered Pharmacist license required. • Bachelor of Science in Pharmacy required; PharmD or MS preferred. • 2+ years of recent acute-care experience OR 1+ year of home infusion experience. • Strong knowledge of IV admixture and intravenous therapy. • Strong clinical judgment and medication-management skills. • Ability to supervise pharmacy support staff. • Strong communication, organization, and time-management skills. • Scheduling flexibility for on-call responsibilities.COMPENSATION & BENEFITS• $124,800–$145,600 annual salary. • Benefits starting Day 1. • 401(k) with 4% company match. • PTO, floating holidays, and paid holidays. • Company-paid life insurance and short-term disability. • Employee Assistance Program. • Learning and development programs. • Employee discounts and referral program.WHY JOIN?• Join a leading home infusion organization serving patients across 40+ states. • Provide meaningful clinical care outside the traditional hospital setting. • Competitive salary with Day 1 benefits. • Collaborative environment focused on specialized infusion care.Type: Direct HireCategory: HealthcareDate Posted: 2026-09-08
