Provide hands‑on care: assist clients with personal hygiene, mobility, light exercise, meals, housekeeping, errands, and medication reminders. Ensure quality & safety: complete home‑safety assessments, conduct routine quality‑assurance visits, and document plan‑of‑care updates for the Care Manager. Train & onboard caregivers: deliver one‑on‑one coaching, skills refreshers, and ongoing competency checks. Serve on the on‑call rotation: triage caregiver call‑outs, fill urgent shifts, and resolve client concerns after hours. Problem‑solve in real time: address changes in client condition, elevate issues, and keep families informed.
