Description:

The RN implements professional nursing practice centered on the patient as a whole person. The creation of a caring environment focused on healing and wellness is essential. Nursing practices are based on evidence, current research, hospital policies, procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Coordinating, ensuring, and providing direct patient care activities on the unit includes provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is part of the role. Providing patient and family-centered education is crucial. Care coordination and individualized treatment planning are applicable tasks. Assessment of patient needs is performed according to RN scope of practice and hospital policies. Consideration of the patient's culture, age, and developmental level is vital. Identifying patient risk factors and presenting complaints is required. Obtaining patient health information from family or others as necessary is part of the process. Collecting objective and subjective data is essential. Verifying initial admission assessment is important if obtained by other team members. Developing a problem list prioritizes patient needs based on input from the inter-professional team. Designing actions and interventions relevant to the patient's culture and age is necessary. Determining desired outcomes in coordination with the patient and family according to the medical plan of care is expected. Fully implementing patient care therapies and procedures with empowerment and clinical autonomy within scope of practice is crucial. Explaining procedures, treatments, and processes to the patient and family is part of the job. Initiating action to alleviate patient stress and discomfort is imperative. Recognizing, responding to, and communicating changes in patient condition appropriately is a key responsibility. Conducting and documenting ongoing assessment and response to procedures and therapies using the nursing process according to hospital policy is required. Delegating tasks appropriately and overseeing team members in the performance of patient care activities is part of the role. Monitoring the accuracy and completeness of delegated patient care provided by other team members is essential. Identifying and addressing educational needs of patients related to prioritized problems according to their level of comprehension is necessary. Facilitating the patient discharge process by executing discharge orders in a timely manner is expected. Providing input into patient/nurse assignments based on staffing, unit activity, and patient acuity is required. Demonstrating active participation in unit-based activities supports organizational values and priorities. Promoting a positive group dynamic and healing environment through words and actions is essential. Assuring nursing excellence in accordance with accepted standards and evidence-based practice is a priority. Actively participating in quality improvement activities and core measures is expected. Using a holistic perspective regarding patient care is necessary.
