Triage RN Overnight opening 5:00pm to 8:00am Thursday - Sunday.
About the Job
As an On‑call/Triage Registered Nurse , you'll be the clinical lifeline for homecare and hospice patients during non‑business hours. You'll triage after‑hours calls, provide timely clinical interventions, make in‑home visits when needed, and coordinate care transitions to ensure safety, comfort, and continuity. This role blends autonomy, critical thinking, and relationship‑centered care—empowering you to prevent unnecessary hospitalizations, support families in moments of uncertainty, and uphold the highest standards of home‑based care.
What You'll Do
- Triage after‑hours calls using established protocols to promote positive outcomes and reduce preventable ER/hospital admissions.
- Assess patient needs and make in‑person visits as necessary, including admissions and routine visits.
- Communicate with physicians and hospice medical directors regarding status changes and to obtain orders.
- Coordinate transitions to inpatient settings or hospice house, providing complete SBAR and medication reports.
- Use disease‑management principles, OASIS knowledge, clinical judgment, and the plan of care to guide after‑hours decision‑making.
- Review after‑hours referrals, assign staff appropriately, and communicate updates to all disciplines and scheduling teams.
- Document all calls, communications, and clinical actions in real time in the patient record and triage log.
- Provide exceptional customer service to patients, families, caregivers, and partners.
- Participate in staff meetings, in‑services, and other agency activities.
- Perform additional duties within scope as assigned.
Who You Are
- A Registered Nurse licensed and in good standing with the New Hampshire Board of Nursing.
- Equipped with at least two years of home health experience and prior triage experience.
- Able to work independently, using strong critical thinking and sound clinical judgment.
- Certified in CPR/BLS and able to meet driving, background, and competency requirements.
- Skilled in communication, patient assessment, and care coordination.
- Comfortable managing complex situations, collaborating across disciplines, and supporting patients and families after hours.
- Preferably experienced in homecare or hospice, with recent triage experience and prior on‑call responsibilities.
- Compassionate, adaptable, and committed to delivering high‑quality, patient‑centered care.
