Outpatient Physical Therapist

This is a 13-week contract position.

32-40 hours per week, Monday through Friday.

The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans based on the patients' needs and goals. This role involves delegating therapy services and ensuring the design of effective functional treatment programs to facilitate patients' return to community activities. The therapist serves as an educator for patients, families, and students while upholding the organization's mission and compliance standards.

Responsibilities:

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop and progress treatment plans focused on discharge goals within the established length of stay.
• Document medical records accurately and in compliance with state and federal guidelines.
• Ensure care is aligned with physician orders and within insurance approvals.
• Participate in student programs and provide a positive learning environment.
• Create individualized professional development plans and pursue ongoing education.
• Adhere to state practice acts and maintain professional licensure.
• Engage in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience is required.
• Active CPR certification.
• Active physical therapy license for the state where work will be performed.
