About the Role:

• As an LPN Licensed Practical Nurse at Milton Nursing and Rehab Center, you will be responsible for providing direct patient care, administering medications, and performing various medical procedures under the supervision of our experienced nursing staff.

• You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, monitoring resident progress and implementing changes as needed.

• You will also be responsible for maintaining accurate and up-to-date resident records, communicating effectively with families and healthcare professionals, and demonstrating compassion and empathy in your interactions with our residents.

Responsibilities:

• Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living (ADLs)

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor resident progress, identifying and reporting changes, concerns, or needs to the team

• Maintain accurate and up-to-date resident records, including medical charts, medication records, and incident reports

• Communicate effectively with families, healthcare professionals, and other staff members

• Participate in quality improvement initiatives, including identifying and implementing best practices

Requirements:

• Current LPN license in the state of Pennsylvania

• Minimum 0 years of experience (new graduates welcome to apply)

• Ability to lift 50 pounds and move residents with assistance

• Basic Life Support (BLS) certification
