About the Role We are seeking dynamic Nurse Practitioners to revolutionize healthcare delivery by conducting virtual wellness visits directly to patients in their homes. As a Nurse Practitioner with Sprinter Health, you will offer a range of healthcare services, which may include virtual adult and/or pediatric wellness visits, health assessments, and related activities. Successful candidates will have experience performing wellness visits and demonstrate dependability, professionalism, problem-solving abilities, and a commitment to delivering exceptional customer service. Responsibilities Review daily case load and ensure access to the necessary tools to service patients. Navigate schedules with modern technology and coordinate with the on-site clinical team (Sprinters) visiting patients at home. Establish warm, welcoming connections with patients and gather relevant information to perform services. Conduct proactive health risk assessments, medication reviews, cognitive screenings, and provide educational information on health and well-being. Collaborate with Sprinters to evaluate vital signs, arrange blood draws, and address patient needs. Provide comprehensive care coordination and management, including preventive care interventions, medication management, referrals to specialists, community resources, and documentation of findings. Qualifications Board Certified as a Family Nurse Practitioner. Active Family Nurse Practitioner license in California. Credentialed Medicaid provider for California (Medi-Cal). Professionalism, integrity, confidentiality, and compassionate care. Willingness to adapt in a dynamic environment and perform problem-solving as needed. Technologically proficient with laptops or mobile devices for charting and HIPAA-secure messaging. Strong written and verbal communication skills. Ability to work independently or in a team.
