• Monday - Friday 7:30am - 4pm with 30 minute lunch break
• 14 patients per day in VA setting
• Outpatient clinic setting
• Team-based approach with nurses, pharmacists, social workers
• Adults only patient population
• Complex chronic conditions including diabetes, heart disease, COPD
• Mental health factors including PTSD, depression, anxiety
• No call and no weekends required
• Paid malpractice insurance; pre-paid travel and housing expenses
