We are seeking a dedicated Wound Care / Admissions Nurse (RN or LPN) to join our clinical team. This role combines specialized wound care with admissions responsibilities, providing an opportunity to make an immediate impact on residents from the moment they arrive at our facility.

What You’ll DoWound Care

• Perform comprehensive wound and skin assessments upon admission and throughout the resident’s stay

• Develop, implement, and monitor individualized wound care plans according to physician orders and facility protocols

• Provide wound treatments and preventive skin care

• Conduct regular rounds to monitor wound progression and overall skin integrity

• Identify changes in wound status and promptly communicate concerns to the appropriate clinical team members and providers

• Collaborate with nursing, dietary, therapy, physicians, and other members of the interdisciplinary team to support wound healing

• Monitor nutrition and hydration needs related to wound healing and communicate concerns as appropriate

• Maintain accurate wound assessments, treatment records, and clinical documentation

• Ensure wound care supplies and treatment areas are organized, clean, and properly stocked

• Follow infection prevention, safety, and wound care best practices

Admissions

• Assist with the admission process for new residents

• Complete nursing assessments and obtain accurate clinical information upon admission

• Review hospital and medical records to identify diagnoses, medications, treatments, wounds, and other clinical needs

• Assess residents for skin integrity and identify existing or potential wounds upon admission

• Communicate admission findings and care needs to the interdisciplinary team

• Help ensure physician orders, medications, treatments, and care plans are accurately implemented

• Provide education and support to residents and families during the admission process

• Assist with coordinating a smooth transition from the hospital or other healthcare setting into the facility

• Ensure required admission documentation is completed accurately and timely

What We’re Looking For

• Current RN or LPN license in the state of practice

• Previous clinical nursing experience required

• Wound care experience strongly preferred

• Long-term care or skilled nursing experience preferred

• Experience with admissions is a plus

• Current CPR certification

• Strong clinical assessment and documentation skills

• Knowledge of wound prevention, treatment, and infection control practices

• Excellent communication and organizational skills

• Ability to prioritize multiple clinical responsibilities

• Compassionate, dependable, and resident-focused approach

• Ability to work independently while collaborating effectively with an interdisciplinary team
