Overview Now Hiring: Care Managers. Join LIFEPlan COO NY for a Care Manager position that serves individuals with intellectual and developmental disabilities (IDD). You will be the trusted partner for individuals and families navigating complex systems of support, lead the development of personalized Life Plans, and connect members to essential services. Location: Clifton Park Hub, Clifton Park, NY 12065. This is not a remote position; it is a flexible work environment with a blend of work from home, field work, and regional travel as well as in-office expectations. Education Level: 4 Year Degree. Responsibilities Deliver person-centered care management services in compliance with regulatory standards and the agency’s quality management plan, policies, and standard operating procedures. Complete comprehensive assessment/reassessment processes and identify gaps in service provision; advocate to reach members’ goals. Develop, implement, and monitor member Life Plans within required timeframes, leading an interdisciplinary team planning process with the person at the center. Address conflicts in the person-centered planning process with the interdisciplinary team and resolve them in a timely manner. Complete all required service documentation within stated timeframes and ensure billing documentation is accurate and valid. Maintain continued eligibility for care management through annual Level of Care determinations and HCBS waiver enrollment; ensure OPWDD eligibility. Identify and access benefits (Medicaid, Social Security, SNAP, etc.) and maintain existing benefits. Maintain current information sharing consent in the electronic health record and update as needed. Coordinate access to high-quality healthcare services (medical, behavioral health, specialized services) with regular communication and follow-up on critical needs. Coordinate preventative and health promotion services as needed and manage transitions of care, including discharge planning and transfers between settings. Foster self-determination and community inclusion through linkage to community resources aligned with member goals and abilities. Utilize health information technology and maintain accurate electronic health records; support telehealth services as needed. Support member self-advocacy using a person-centered, strength-based approach and advocate as necessary. Attend department/team meetings, trainings, and supervisions; complete required trainings within timeframes. Travel throughout the designated service area to meet with members and providers; ensure needs are met and follow up as required. Identify and follow incident reporting guidelines to ensure member safety; adhere to all policies and procedures for care management and related functions. Maintain confidentiality in accordance with HIPAA and privacy practices. Perform other duties as assigned. Must possess a valid New York driver’s license or license from a contiguous state and have a dependable vehicle for frequent travel. Qualifications Bachelor’s degree (BA/BS) with two years of relevant experience, or RN license with two years of relevant experience, or master’s degree with one year of relevant experience. Degrees in Health and Human Services, Psychology, Sociology, or related fields are preferred; experience with IDD, case management, mental health, or substance abuse is helpful. Ability to meet members in homes or community locations; travel to off-site locations required; comfortable driving. Must reside in New York State or a contiguous state within 100 miles of the assigned office hub. Proof of valid auto insurance required at time of hire. Excellent interpersonal, public speaking, and written communication skills; ability to work autonomously and in a team; strong integrity and commitment to serving people with IDD and families. Additional Information This role is not remote. It requires a mix of on-site, field work, and regional travel. Most hours are Monday–Friday with potential for evenings and weekends as deadlines arise.
