New York Care Specialist

To support the Care Management team, the remote New York Care Specialist will execute non-clinical aspects of care management, including data entry, member outreach, and coordination of care plans while working collaboratively with clinical staff. Key responsibilities Perform accurate intake and data entry for Care Management requests and referrals across various communication channels Conduct telephonic outreach to members for program enrollment and support their ongoing care needs Collaborate with the interdisciplinary team to manage caseloads and update care plans for low and moderate risk members Required qualifications Bachelor's Degree Medical Assistant certification preferred 1 - 2+ years of client-facing or data entry experience in a healthcare environment Proficiency in MS Office applications Knowledge of medical terminology and medical payment processes
