Seven Healthcare are seeking an experienced Physical Therapist for a contract assignment in Reidsville, NC. This role involves providing comprehensive home health services and working closely with patients to develop and implement effective treatment plans. - Regularly reassess and modify treatment plans as necessary
- Communicate patient needs and progress with the treatment team and family
- Consult with medical providers about patient progress and discharge
- Assist in procuring and incorporating adaptive equipment to enhance rehabilitation
- Maintain and submit required documentation per regulations
- Supervise Physical Therapist Assistants and Aides
- Participate in staff development programs and continue education
- Licensed NC PT or compact license
- Home health experience
- Experience with HCHB and OASIS
