Responsibilities
- Administer prescribed medications and treatments per physician orders; document accurately
- Monitor residents’ health status, perform routine assessments, and record vital signs
- Provide essential nursing care, wound management, and assistance with activities of daily living
- Maintain clear, timely documentation in the resident care records; ensure regulatory compliance
- Collaborate with the interdisciplinary team to update and implement care plans
- Uphold patient safety, infection control practices, and privacy standards

Requirements
- Active LPN license to practice in New Hampshire
- Current CPR/BLS certification
- Prior long-term care or skilled nursing experience preferred
- Strong communication and documentation skills; ability to work in a team
- Availability for evening shifts and a 10-week travel assignment
- Ability to work effectively in a long-term care setting and meet assignment requirements
