Develop, implement, and adjust the patient’s plan of care in collaboration with the interdisciplinary team. Utilize therapeutic modalities and rehabilitation equipment; apply splints, orthoses, prostheses, wheelchairs, and ADL equipment as appropriate. Document assessments, interventions, and outcomes; ensure compliance with home health documentation standards and regulatory requirements. Educate patients and families on safety, exercise programs, activities of daily living, and caregiver techniques. Coordinate care with other providers; participate in care conferences and contribute to the overall patient care plan. Travel to patient homes and maintain appropriate equipment; manage time effectively in a home-based setting.
