We are seeking a Board Eligible orBoard Certified Family Medicine or Internal Medicine Physician to become partof a collaborative care team focused on improving health outcomes throughpreventive care, chronic disease management, and coordinated treatment.

Practice alongsideexperienced Nurse Practitioners, Care Managers, Pharmacists, Social Workers,and Clinical Support Teams in a supportive environment designed to helpphysicians focus on delivering exceptional patient care.

Key Responsibilities:

• Deliver comprehensiveprimary care services for adult patients.
• Diagnose and manage acute,chronic, and complex medical conditions.
• Interpret diagnostic testsand coordinate specialty referrals when appropriate.
• Collaborate withinterdisciplinary care teams to optimize patient outcomes.
• Promote preventive care,wellness, and chronic disease management.
• Participate in qualityimprovement initiatives and patient engagement programs.
• Maintain accurate and timelyelectronic medical record documentation.

Qualifications:

• MD or DO from an accreditedmedical school
• Active Arizona medicallicense (or eligibility to obtain)
• Current DEA registration
• Board Certified or BoardEligible in Family Medicine or Internal Medicine
• Passion for deliveringvalue-based, patient-centered care
• Excellent communication andcollaborative care skills
