Speech Therapist

Stamford Nursing and Rehabilitation - This position is for a skilled nursing facility under new ownership in Stamford. Stamford Nursing and Rehabilitation has both short-term and long-term patients. This facility has an in-house therapy team with smaller daily case loads withan emphasis on optimizing time with patients. We have flexible scheduling! Speech Therapist Job Responsibilities: Develop and implement a Speech Therapy plan of care for facility residents and short-term patients. Assist as needed with patient screens for Speech Therapy. Communicate progress and any barriers toward SLP goals with interdisciplinary team members. Complete timely Speech therapy documentation in compliance with regulatory, licensing, facility, and payor requirements. Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods. Provide and document patient, family, and caregiver training regarding each speech therapy patient’s specific plan of care and recommendations for follow up. Maintain productivity standards as expected by Director of Rehab or Administrator.
