Napa Post Acute Napa, CA $50-$55 per Hour Full-Time Position

What You'll Do

• Conduct comprehensive nutrition assessments for residents upon admission and as needed

• Develop, implement, and monitor individualized nutrition care plans

• Evaluate nutritional status and provide recommendations to support resident health and recovery

• Collaborate with physicians, nursing staff, speech therapists, and dietary personnel to ensure optimal resident outcomes

• Monitor residents for significant weight changes, nutritional risks, and dietary concerns

• Participate in care plan meetings and interdisciplinary team discussions

• Ensure compliance with state, federal, and facility regulations regarding nutrition services

• Provide nutrition education and counseling to residents, families, and staff as needed

• Support the Dietary Services department in maintaining high-quality nutrition programs

Qualifications

• Current Registered Dietitian (RD/RDN) credential required

• Licensed or eligible for licensure in California, as applicable

• Experience in skilled nursing, post-acute care, rehabilitation, long-term care, or healthcare preferred

• Strong knowledge of clinical nutrition, therapeutic diets, and regulatory requirements

• Excellent communication, organizational, and documentation skills

• Ability to work independently while collaborating effectively with an interdisciplinary team
