Experience and Education Required Education • Bachelor’s Degree – Bachelor of Science (BS) degree or • Doctor of Pharmacy (PharmD) degree from an American College of Pharmaceutical Education (ACPE) accredited school of pharmacy Experience • 1 year with experience in a hospital setting required for an inpatient position Licenses and Certifications • Eligible for licensure by Texas State Board of Pharmacy (TSBP) is required and Texas license with TSBP upon hire and within 30 days of date of hire. • ACPE‑IV Sterile Compounding (IV) certification accredited by ACPE upon hire • BLS Basic Life Support certification with a course accredited by the American Heart Association (AHA) or American Red Cross (ARC) upon hire • Advanced Cardiac Life Support course accredited by the AHA or ARC required for inpatient positions within 180 days Preferred Experience • 1 year retail experience for retail position Job Duties Perform job responsibilities in accordance with organizational and departmental policies, procedures, performance improvement objectives and environmental/infection control standards as well as state and federal laws and regulations. Perform all computer order entry accurately, thoroughly and promptly. Review/update medication profiles (therapies, allergies, height, weight, etc.) and be cognizant of patient charges. Review medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety. Seek clarification if necessary and document interventions (ADR). Work independently and troubleshoot with minimal supervision, taking charge when needed and exercising good judgment. Cross‑train and maintain competency in multiple areas. Enforce the formulary system and follow departmental policy when acquisition of a non‑formulary product is judged necessary. Participate in cost‑effective use and selection of pharmaceuticals. Communicate drug information verbally and in writing to physicians, nurses, etc. in a courteous and professional manner. Assist in patient/family education and counseling regarding drug therapy and side effects. Effectively document clinical activities, demonstrating cost savings and concisely conveying key information required for follow‑up by fellow pharmacists. Strive to improve patient satisfaction, as measured by Press‑Ganey scores. Manage technicians and all other supporting personnel. Delegate appropriate/reasonable tasks and validate accuracy of medications intended for distribution from the IV lab, central pharmacy or decentralized automated dispensing machines. Audit and sign out all controlled substances in accordance with state/federal guidelines and organizational policies. Investigate and correct discrepancies and unusual usage patterns/trends. Actively enhance personal growth and development by participating in educational programs, in‑services and meetings. Participate in new employee orientation and training. Willingly take on additional projects as needs develop. Actively participate in the clinical program. Support P&T initiatives by monitoring target drugs for protocol discontinue, substitution, IV to PO, etc. Collect applicable data/measures and communicate opportunities for improvement. Perform therapeutic monitoring of specified drug therapies and recommend dosage adjustments as needed based on renal function evaluation and pharmacokinetic calculations. Duties performed may include one or more of the following core functions: a) Directly interacting with or caring for patients; b) Directly interacting with or caring for human‑subject research participants; c) Regularly maintaining, modifying, releasing or similarly affecting patient records (including patient financial records); or d) Regularly maintaining, modifying, releasing or similarly affecting human‑subject research records. Perform other duties as assigned.
