Arizona Priority Care (AZPC) is an Integrated Provider Network focused on providing Whole-Person care to Senior and Medicaid populations, through advanced value-based models. The clinical Pharmacist at AZPC provides clinical, supervisory and educational functions as necessary to support the overall patient care provided by AZPC. Pharmacists provide medication therapy management for patients as defined under medical staff approved protocols. Pharmacists are responsible to supervise directly the job activities of AZPC support personnel.

POSITION DUTIES & RESPONSIBILITES

• Follows medical staff approved protocols in management of patients defined under such protocols (anticoagulation, hepatitis C, Diabetes, anemia).
• Provides comprehensive medication therapy consults to patients referred to AZPC services.
• Provides general drug information support to AZPC providers.
• Participates in team rounding situations (LAW, home health, case management).
• Provides direct supervision to AZPC support staff (technicians/clerks).
• Provides training/supervision/mentoring to pharmacy students and residents who are in the process of completing pharmacy educational programs.
• Complies with accepted professional standards and principles.
• Participates in organizational activities that promote the organization's role as an effective member of the healthcare delivery system.
• Performs other duties as assigned
• EDUCATION, TRAINING AND EXPERIENCE

• Registered Pharmacist, Masters in Pharmacy or equivalent. Doctorate of Pharmacy preferred. Current AZ License
• Post graduate residency training or equivalent training. Board Certified Ambulatory Care Pharmacist (BCACP) preferred.
• Minimum of 1 year experience in retail or healthcare pharmacy.
• Strong computer skills and ability.
• Able to multi-task effectively to achieve goals.
• Able to work independently and as part of a healthcare team.

• BLS required within 60 days of hire. BLS not required for employees who are not involved in direct patient care.

Current unrestricted AZ license and proof of insurance.

Compensation Range: $72.00 – $77.00/hour

*This role requires FT in-office presence for the first 60 days of employment. Hybrid schedule available after initial training period. *
