**Join Our Team as a Home Health Physical Therapist!** **Job Description:** - Provide exceptional physical therapy services within your state's legal scope of practice. - Maintain valid licensure and certifications in accordance with state and federal regulations. - Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications, ensuring compliance with Medicare, state, and CareM standards. - Develop, implement, and update personalized care plans tailored to each patient's needs and physician orders. - Deliver skilled interventions and maintenance therapy programs as defined by state practice acts. - Supervise Physical Therapist Assistants (PTAs) and Home Health Aides in compliance with state laws. - Collaborate with physicians and interdisciplinary team members to discuss treatment adjustments and patient progress. - Educate patients and caregivers on home exercise programs, mobility techniques, and adaptive strategies for enhanced independence and safety. - Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within specified timeframes. - Ensure accurate and compliant documentation through appropriate application of ICD-10 coding. - Share knowledge through in-service programs and contribute to staff education initiatives. - Engage actively in Quality Assessment and Performance Improvement (QAPI) activities to uphold clinical excellence. **Other Duties:** - Provide care in alignment with infection control, safety, and risk prevention protocols. - Accurately document clinical interventions, care coordination, and timely communication. - Participate in interdisciplinary meetings, case reviews, and continuing education opportunities. - Maintain adherence to HIPAA guidelines and organizational privacy standards. - Represent our organization positively and professionally during home visits and community interactions. - Work independently with minimal oversight while demonstrating initiative in all responsibilities. - Deliver courteous, timely, and professional customer service to patients, families, and partners. - Adapt to shifting responsibilities with flexibility in task completion. - Utilize organizational technology systems effectively for communication, documentation, and data management. - Follow all organizational policies, including job descriptions, mission, and the Employee Handbook. - Communicate and collaborate respectfully in support of team goals. - Perform additional duties as assigned, always prioritizing the organization's best interests. **Qualifications:** - Graduate of an accredited physical therapist curriculum recognized by the Commission on Accreditation in Physical Therapy Education. - Currently licensed as a Physical Therapist in your state of practice. - Two years of clinical experience is preferred; home health experience is a plus but not required. - Ability to exercise sound clinical judgment and work independently in diverse patient care settings. - Strong communication and interpersonal skills, with sensitivity to diverse backgrounds. - Basic computer proficiency and a willingness to learn EMR systems and OASIS documentation. - Current CPR certification is required. - Commitment to compliance with Medicare/Medicaid Conditions of Participation, state regulations, and agency policies. - Reliable transportation, a valid driver’s license, and auto insurance for home visits. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
