Physical Therapist - PRN

Position Summary We are seeking a licensed Physical Therapist to evaluate and treat residents to restore function, improve mobility, relieve pain, and prevent or limit physical disabilities. Responsibilities Evaluate residents’ physical abilities and develop individualized treatment plans. Provide physical therapy treatments including therapeutic exercise, manual therapy, and modalities. Document evaluations, treatment plans, and progress notes accurately. Collaborate with the interdisciplinary care team on resident goals and discharge planning. Supervise and direct Physical Therapy Assistants as applicable. Perform other duties as assigned by facility management. Qualifications Doctorate or master’s degree in Physical Therapy from an accredited program. Current PT license in Tennessee. Long‑term care / skilled nursing experience preferred. Strong communication and interpersonal skills and a genuine interest in working with older adults.
