Work location – Virtual

This role enables associates to work virtually full-time, except for required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development. Alternate locations may be considered if candidates reside within a commuting distance from an office.

Work shift: Four (4) 10-hour shifts weekly, with every other weekend required (Both Saturday & Sunday). The alternating Saturday/Sunday are counted towards the 4 shifts each week.

Work hours: 8am-7pm CST

The LPN/LVN is responsible for the examination and treatment of patients under the direction of the physician.

Primary duties may include, but are not limited to:

• Outreach to our members who have been recently discharged from the ER to conduct follow-up and refer to CareBridge services.
• May enroll members into CareBridge services and schedule visits with nurse practitioners.
• May triage members, conduct nursing assessments and refer to acute nurse practitioner.
• Reviews patient medical records, interviews patients and measures vital signs.
• Explains procedures, performs treatments, and administers prescribed medication.
• Facilitates patient transfers to alternate level of care.

Minimum requirements:

• Requires an LPN or LVN and minimum of 2 years of experience as an LPN/LVN; or any combination of education and experience, which would provide an equivalent background.
• Current LPN/LVN license in the applicable state required.
• For Carelon Health business unit, satisfactory completion of a Tuberculosis test is a requirement for this position.

Preferred qualifications, skills, and experiences:

• Bi-lingual (English/Spanish) preferred.

• Previous experience in behavioral health, care coordination, community health, transitional care and/or chronic disease management is strongly preferred.

For candidates working in person or virtually in the below location(s), the salary* range for this specific position is $22.30 - $40.15/hour
