We’re seeking a full-time Wraparound Facilitator to become an essential part of a caring, community-focused team that transforms lives by delivering services that empower youth and families to overcome challenges and build brighter futures. Your Role in Our Mission As a QP Facilitator on the High-Fidelity Wraparound team, you will build and lead the Child and Family Team (CFT) to support youth on a mission to stay "at home, in school, and out of trouble." You will coordinate team activities, provide direct support and education to families and team members, guide families to accomplish objectives outlined in their Family Plan, and ensure accurate documentation in the UNCG database. Schedule & Service Area Full-time Monday-Friday position with a flexible schedule primarily during business hours; includes 3 pm - 7 pm as needed. Service area: Davie County, Davidson County, and surrounding areas.
