Temp - PT Rehabilitation - Outpatient Physical Therapist (Days) Windsor VT

Responsible for providing PT treatments to employees/patients based on an evaluation of their need and status. Maintains appropriate records and assists with in-services and Community Health Program.

Requested time off during the course of this assignment MUST be noted and verified at upload. RTO REQUESTS AFTER UPLOAD WILL NOT BE APPROVED!!

Traveler must be willing to work Thanksgiving Holiday (and Friday and weekend after If Needed/Scheduled). RTO during these holidays will not be approved. NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better). Thanks!

Local Travelers - Those who reside within 50 miles of the facility - are not eligible for this need.

Non-Billable Orientation: 8hrs
Holiday and OT multipliers: 1.25%
Call, Call-Back and Charge: Not applicable for this need

Requirement description :

• 2+ years current PT experience - Required
• Ability to work independently - Required
• Experience in a Critical Access hospital - Highly Preferred
• Travel experience - Preferred

• 1 Supervisory Signed Reference (from within the past two years of employment)
• License/Certifications - Must be uploaded with profile (VT License or Compact, BLS - Facility will not pend license/certs)
