Gather and compile input from all disciplines during treatment planning meetings to produce written Person‑Centered Recovery Plans (PCRP). Collect, synthesize, transcribe, and enter data into the medical record documenting all treatment recommendations and interventions. Develop and maintain a tracking system to ensure treatment/recovery plans are completed in a timely manner and updated as needed. Coordinate, schedule, and attend treatment team meetings; maintain electronic and paper files. Serve as a liaison for program staff, patients, and centralized rehabilitation staff, ensuring PCRP interventions are documented. Participate in mandatory training and any required program or specific training to maintain skills and knowledge. Assist in disaster response and/or recovery or Continuity of Operations (COOP) activation, which may require alternate shift patterns or location. Perform other duties as assigned, including work on evenings, nights, and weekends as eligible for additional pay. Essential Job Functions (EJFs) Attend work on a regular basis and may be required to work specific shift schedules, rotating schedules, extended shifts, and/or overtime. Enter data into the medical record for comprehensive treatment/recovery plans and updates, ensuring accuracy, timeliness, and compliance with CMS and TJC regulatory standards. Coordinate with treatment team members to complete and update plans according to policy. Maintain tracking systems and administrative files, ensuring all documentation is person‑specific and current. Act as liaison/contact person to facilitate communication and documentation of PCRP interventions. Participate in disaster response and/or COOP activation when required. Knowledge, Skills, and Abilities (KSAs) Knowledge of office practices and administrative procedures. Knowledge of HIPAA and confidentiality rules. Knowledge of the Electronic Medical Record (EMR). Proficient computer skills and written communication, including medical and psychiatric terminology. Skill in using standard office equipment and software. Ability to synthesize large amounts of information into concise, clear documentation. Ability to coordinate team members to finalize plans and reviews per policy. Ability to be productive and responsible in a professional environment. Ability to communicate effectively both orally and in writing. Ability to multitask and work under pressure. Initial Screening Criteria Graduation from a standard senior high school or equivalent, OR At least two (2) years’ experience in a professional support role. Additional Information Applicants must pass a fingerprint criminal background check, pre‑employment drug screen, and registry checks including CANRS, Nurse Aid, Medication Aide, Employee Misconduct, and LEIE. Males 18‑25 must register with Selective Service. All applicants must be at least 18 years of age to be considered for employment at a state‑operated facility; Waco and Terrell Center for Youth applicants must be 21 years of age. Flexibility in work hours may be required; overtime and extended hours may apply.
