On Call Registered Nurse

Heartland Hospice is seeking an On Call Registered Nurse to provide after-hours hospice care and support to patients and families in home and facility settings. The RN will triage calls, make urgent visits, manage symptoms, perform nursing assessments, and implement care plans focused on comfort and dignity. Responsibilities include coordinating with the interdisciplinary team, communicating with physicians, documenting timely and accurately, educating families on end-of-life care, and ensuring compassionate, patient-centered service aligned with our mission-driven, collaborative culture.
