Role Overview: A licensed Physical Therapist Assistant (PTA) works closely with a supervising Physical Therapist to deliver individualized treatment plans for older adults through FOX's Geriatric House Calls™ model, providing hands‑on care in patients' homes and senior living communities. Key Responsibilities Implement treatment plans, adjusting activities based on patient response Guide patients through therapeutic exercises and functional activities Accurately document visits within the EMR system Maintain communication with patients, POAs, and referral sources; assist with coverage coordination Qualifications & Skills Valid PTA license and current CPR certification Graduate of an accredited PTA program Basic computer proficiency Ability to perform physical tasks such as lifting, transferring patients, and extensive mobility Key Attributes & Environment: The role emphasizes one‑on‑one patient care, interdisciplinary teamwork with PTs, OTs, SLPs, and Fitness Specialists, and offers mentorship with growth opportunities. PTAs build lasting relationships, contribute to independence and wellness, and work within a supportive, community‑focused culture. #J-18808-Ljbffr
