Responsibilities
- Conduct initial OT evaluations and ongoing reassessments for residents in the skilled nursing setting
- Develop and implement individualized treatment plans to maximize independence in activities of daily living and functional mobility
- Administer therapy sessions, monitor progress, and adjust goals as needed
- Document assessments, treatment plans, progress notes, and discharge summaries per regulatory requirements in the electronic health record
- Collaborate with an interdisciplinary team (PT, SLP, nursing, social work) to coordinate care and discharge planning
- Ensure adherence to state and federal regulations, payer requirements, and facility policies

Requirements
- Active OT license in good standing with eligibility for NM licensure or current NM licensure
- Master's or Doctorate degree in Occupational Therapy from an accredited program
- NBCOT certification preferred or eligible
- Minimum 1 year of OT experience preferred, ideally in SNF or geriatric settings
- Ability to work effectively in a skilled nursing facility environment and within a travel assignment
- Strong interpersonal and documentation skills; ability to collaborate with an interprofessional team
