Key Responsibilities

• Deliver On-Site Therapy: Provide individualized, on-site therapy services to residents within various assisted living facilities, ensuring the highest standard of care.
• Conduct Evaluations: Perform thorough evaluations to assess residents' physical abilities and limitations, leading to the creation of personalized treatment plans informed by orthopedic best practices.
• Collaborate with Staff: Work closely with nursing staff, caregivers, and our internal team, including MDs and Occupational Therapists, to ensure seamless care coordination.
• Educate Residents and Families: Offer guidance and education on safe mobility, home exercise programs, and overall wellness to enhance residents' quality of life.
• Utilize Technology: Implement mobile electronic medical record (EMR) systems for efficient, HIPAA-compliant documentation that supports streamlined communication and oversight.
• Liaison Role: Act as a crucial point of contact between the facilities and our multidisciplinary clinic for referrals, follow-ups, and treatment progress updates.

Patient Population / Client Focus

In this role, you will primarily work with elderly residents who may be managing a variety of orthopedic conditions commonly seen in aging populations, such as arthritis, joint pain, and post-surgical rehabilitation needs. Your focused approach will help improve their mobility, balance, and overall physical function, fostering independence and enhancing their quality of life.

Qualifications

• Must hold a valid Physical Therapist license in the State of New Jersey, or be eligible for licensure.
• A bachelor's degree in Physical Therapy or an equivalent program is required.
• Exceptional interpersonal and communication skills to interact effectively with patients, families, and interdisciplinary teams.
• A strong clinical understanding of orthopedic conditions prevalent among aging adults.
• Ability to work autonomously in residential settings and adapt to various environments.

Preferred Experience

• Minimum of two years of experience in outpatient, geriatrics, or assisted living facilities, skilled nursing facilities (SNFs), or home care settings is preferred.
