Role Description The Family Nurse Practitioner is a full-time, on-site role based in Charleston, SC. This role includes providing comprehensive primary care services such as health assessments, chronic disease management, preventive care, and acute visit evaluations within the Direct Primary Care model. The practitioner will conduct in-person visits, review patient histories, order and interpret diagnostic tests, prescribe medications, and coordinate care with other healthcare professionals as needed. Day-to-day responsibilities also include patient education, care planning, and documentation in the electronic health record to ensure continuity and quality of care. The practitioner will collaborate with the clinical team to support same-day and next-day access, contribute to practice workflows, and uphold the center’s standards for patient-centered, transparent care. Qualifications Demonstrated clinical expertise as a Family Nurse Practitioner with strong foundations in Nursing and Primary Care. Proficiency in delivering evidence-based Primary Care services to ALL ages in an outpatient setting. Active, unencumbered Nurse Practitioner license in South Carolina and national board certification (FNP-C or FNP-BC). Experience or strong interest in Direct Primary Care, value-based care, or membership-based primary care models. Excellent communication, patient education, and interpersonal skills, with a commitment to inclusive, patient-centered care. Ability to work independently in a small-team environment, manage a varied patient panel, and adapt to evolving practice needs. Current DEA registration, BLS certification, and familiarity with electronic health records. Prior outpatient primary care or family practice experience preferred; experience with employer-sponsored healthcare programs is a plus.
