In this role, you will assess patients in both inpatient and outpatient settings, crafting personalized treatment plans that cater to each individual's journey. Your hands-on interventions will focus on enhancing mobility, strength, balance, and overall function as you collaborate intimately with an interdisciplinary team, including nurses, physicians, case managers, and other rehab professionals, to optimize patient outcomes. Moreover, your role extends to empowering patients and their families through education on therapeutic goals, discharge planning, and home exercise routines. You will meticulously document evaluations, treatments, and progress, ensuring compliance with all facility policies and regulatory standards. Additionally, your active participation in care team rounds and conferences will be pivotal in fostering coordination and continuity in patient care.

To qualify for this role, you must hold a Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program, along with valid state licensure as a Physical Therapist. CPR certification is preferred, and any specialized certifications (such as those in geriatric or neurological therapy) will be valuable, although they are not mandatory.
