PRN Physical Therapist (PT) – CareCore at Willowood

CareCore at Willowood is seeking a compassionate and dedicated PRN Physical Therapist (PT) to join our exceptional in-house rehabilitation team. This is an excellent opportunity for a therapist looking for flexible scheduling while making a meaningful impact in the lives of residents in a skilled nursing and long-term care setting.

• PRN (as-needed) schedule

• Flexible weekday and weekend opportunities

• In-house rehabilitation program

• Skilled Nursing & Long-Term Care setting

• Collaborative interdisciplinary team

• Opportunity to pick up additional hours as available

• Perform comprehensive physical therapy evaluations

• Develop and implement individualized treatment plans

• Provide evidence-based therapy services to help residents improve mobility, strength, balance, and functional independence

• Collaborate with physicians, nursing staff, therapy team members, residents, and families

• Complete accurate and timely documentation using Net Health

• Utilize PointClickCare (PCC) as part of the interdisciplinary care process

• Maintain compliance with all federal, state, and company regulations

• Current Ohio Physical Therapist (PT) license in good standing

• Graduate of an accredited Physical Therapy program

• Skilled nursing or long-term care experience preferred, but not required

• Excellent communication, documentation, and clinical decision-making skills

• Experience with Net Health and PointClickCare (PCC) is preferred

• Flexible PRN scheduling

• In-house therapy department focused on quality patient outcomes
