Pharmacist

Responsibilities Under general direction, the Clinical Pharmacist performs professional pharmaceutical duties in drug education and utilization, counsels patients on drug usage, educates health care personnel on drug utilization, and may compound drugs and dispense medicines in a clinical pharmacy. Interviews and examines patients for drug evaluation; checks drug adherence and side‑effects; discusses symptoms and effects of prescribed drugs with patients and maintains a drug history. Consults with physicians on drug usage and the drug regime for specific patients, especially hypertensive, diabetic, and psychiatric patients; recommends changes in drugs or dosages. Provides formal and informal training for nursing staff, medical and pharmacy students and residents and other health care staff on drug utilization, dosage, incompatibilities, preferred dosage forms, evidence‑based medication therapy and toxicity, etc. Participates in the development and revision of medication use policies and guidelines. Compounds drugs, prepares, and dispenses medications, checks pharmacy stock, and maintains records. Minimum Qualifications Valid Registered Pharmacist license issued by the California State Board of Pharmacy. Completion of six (6) months of a pharmacy residency program; OR Completion of six (6) months of a post‑graduate pharmacy fellowship program; OR Two (2) years of professional level experience as a Clinical Pharmacist equivalent to class 2454 Clinical Pharmacist within the City and County of San Francisco; OR Possession of a Board Certification in a pharmacy specialty area. Desired Qualifications Demonstrated experience with the acute care medicine patient population. Clinical and/or lived experience with diverse communities. Excellent oral and written communication and interpersonal skills. Completion of an ambulatory/primary care pharmacy residency, specialty residency, or fellowship. Fluency in Spanish and/or Cantonese is highly desired. Mission‑driven, passionate candidates with Primary Care clinical expertise and lived experience with Black/African American, Latinx, LGBTQIA+ and other diverse and intersecting communities. Meets requirements for prescribing as required by the Standardized Procedure for Ambulatory Care Pharmacist within San Francisco Health Network. Completion of a psychiatric pharmacy residency. Clinical experience working in substance use disorder or mental health settings. Board Certification as a Psychiatric Pharmacist. Possession of a valid Advanced Practice Pharmacist license in California. Completion of a geriatric pharmacy residency. Certification in Geriatric Pharmacy.
