About the Position

Job Title: Home Health RN Profession: Registered Nurse Specialty: Home Health Duration: 13 weeks Shift: 8H Days Hours per Shift: 8 hours Experience: Minimum 2 years License: Valid driver's license and insurance required Certifications: BLS required Must-Have: Experience with HomeCare HomeBase preferred. Description: The Home Health RN will provide care to patients recently discharged from the hospital. The patient population is at high risk for rehospitalization. Common diagnoses include CHF, COPD, and various wounds, including some with wound vacs and IV-PICC lines. Typical patient education involves the use of respiratory equipment such as vents, CPAP, and BiPap. The RN will see approximately 5 patients per day. The visit types include start of care, recertification, resumption of care, and routine visits. There will be opportunities for collaboration with a home health aide in the department. The schedule is organized over five days a week, with changes frequently occurring due to unforeseen circumstances. Weekend shifts may be assigned, with flexibility around the schedule. Holiday coverage will be needed, particularly for Thanksgiving and Christmas. This position does not have a call or standby requirement. The dress code is work-appropriate scrubs.
