TITLE
Physical Therapist - Rehabilitation

SCHEDULE
Days (8:00 AM - 4:30 PM), no weekends

ASSIGNMENT DETAILS
Contract Length: 13 weeks
Guaranteed Hours: 40

REQUIREMENTS
- Current unencumbered state Physical Therapy license required.
- Healthcare Provider Level BLS certification required.
- Must be able to travel to multiple points of care.

ROLES & RESPONSIBILITIES
- Evaluate and assess patients with injuries or conditions affecting physical mobility and functional independence.
- Develop and implement individualized treatment plans aimed at improving functional outcomes.
- Collaborate with interdisciplinary teams to facilitate comprehensive patient care.
- Provide education and support to patients and their families regarding rehabilitation and recovery processes.
- Monitor and document patient progress and adjust treatment plans as necessary.
