Admission/charge nurse (LPN) – Full‑time, salary $35–$48 per hour (DOE). Location: Avamere Rehab of Cascade Park, 801 SE Park Crest Ave., Vancouver, WA 98683. Avamere Rehab of Cascade Park is an 88‑bed skilled nursing community located in Vancouver, Washington. Responsibilities Complete required record keeping for admission, transfer, and discharge of residents. Coordinate daily with nursing staff and support personnel to plan shifts, services, programs, and activities to maintain each resident’s care regimen. Prepare and administer medications; review medication cards for completeness and accuracy. Plan, organize, develop, implement, and interpret programs, goals, objectives, policies and procedures necessary for providing quality care. Provide leadership to nursing personnel and conduct daily rounds to ensure staff follows resident care plans and wishes. Participate in facility surveys conducted by authorized government agencies. Qualifications Must possess a nursing degree from an accredited college or university. Possess a current, unencumbered, active license to practice as an LPN in this state. Knowledge of nursing and medical practices and procedures, as well as laws, regulations and guidelines pertaining to nursing care facilities. Experience with Electronic Medical Records and computer documentation systems. Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
