Case Worker (Full-Time – 35 Hours/Week) Shift Schedule: Monday–Friday, 9:00 AM – 5:00 PM As a Case Worker, you will provide critical support to older adults and their families through in-home and in-office assessments. Under the guidance of Master’s-level staff, you will develop comprehensive care plans, advocate for essential benefits, and help seniors maintain their autonomy and well‑being within the community. What You’ll Be Doing Responsible for the full spectrum of social services, you will ensure clients receive the clinical and practical support necessary to navigate the challenges of aging. Key Responsibilities Assessment & Counseling: Conduct in‑home and in‑office assessments (utilizing PHQ‑9, NSI, etc.) and provide individual/family counseling to resolve complex social and emotional challenges. Case Management: Develop and implement written case plans, monitoring service efficacy and revising plans as client needs evolve. Advocacy & Benefits: Screen for eligibility and advocate for clients to secure benefits including Medicare, Medicaid, public assistance, and SNAP. Direct Service Coordination: Arrange for essential services such as home care, meals on wheels, legal aid, and psychiatric therapy. Financial Protection: Evaluate clients’ capacity to manage affairs, helping protect them from financial exploitation and assisting with guardianship or money management arrangements. Community Engagement: Lead group counseling sessions, caregiver support groups, and represent JASA at community conferences and meetings. Reporting: Maintain accurate case records and statistics utilizing program‑specific software. What You’ll Need Education: Graduate of an accredited college or university with a BSW, BA/BS, or equivalent degree. Skills: Strong writing and record‑keeping skills; must be computer literate. Communication: Proficiency in English required; knowledge of Spanish is helpful. Travel: Ability to travel to clients’ homes via mass transit and navigate stairs or physical barriers.
