Licensed Practical Nurse

DSHS BHHA Lake Burien – Licensed Practical Nurse 2 Join our team at Lake Burien Transitional Care Facility (LBTCF) as a Licensed Practical Nurse 2 and play a vital role in supporting youth on their journey toward stability, recovery, and independence.

In this position, you will provide direct nursing care, administer medications and treatments, monitor patient health and well-being, and support the implementation of individualized treatment plans under the direction of a Registered Nurse. Your work will help ensure youth receive safe, compassionate, and therapeutic care in a structured residential treatment environment while contributing to meaningful treatment outcomes.

Some Of What You'll Do

• Provide direct nursing care to youth residents, including administering medications, implementing physician orders, monitoring health status, performing assessments, and delivering treatments in accordance with nursing care plans.
• Monitor and document patient conditions, interventions, and treatment outcomes using electronic health records and established clinical documentation standards.
• Support the physical, emotional, behavioral, and social well-being of youth through therapeutic interactions, patient advocacy, health education, and participation in treatment planning.
• Lead, facilitate, and participate in recreational, psychoeducational, wellness, and community-based activities that promote patient growth, independence, and recovery.
• Maintain a safe and therapeutic environment by monitoring patient behavior, utilizing de-escalation techniques, responding to crises, and implementing approved safety interventions when necessary.
• Assist youth with activities of daily living, orientation to facility routines, transportation to appointments and activities, and the development of adaptive living skills.
• Collaborate with interdisciplinary treatment teams, families, external providers, and community partners while supporting administrative functions and completing other duties as assigned.

Desired Skills, Knowledge, And Abilities

• Knowledge of nursing principles, behavioral health practices, and trauma-informed care applicable to youth residential treatment settings.
• Skill in administering medications, implementing treatment plans, monitoring patient conditions, and documenting care in accordance with established standards.
• Proficiency in the use of medical equipment, including blood pressure cuffs, stethoscopes, thermometers, glucometers, weight scales, and patient lifting devices.
• Ability to effectively communicate and build positive working relationships with youth, families, healthcare professionals, and other stakeholders.
• Ability to assess patient needs, observe and report changes in condition or behavior, and exercise sound clinical judgment in a fast-paced environment.
• Ability to utilize de-escalation techniques, respond appropriately to behavioral crises, and implement approved safety interventions when necessary.
• Proficiency with electronic health record systems, Microsoft Office applications, and other computer-based programs used to support patient care and documentation.

We are seeking professionals who possess a current Washington State license to practice as a Licensed Practical Nurse (LPN).

Additional Information

• These positions work 10-hour shifts from 6:30 a.m. to 5:00 p.m. on a rotating two-week schedule.
• LPN 1 works Sunday, Tuesday, Wednesday, and Thursday during Week 1 and Monday, Tuesday, Friday, and Saturday during Week 2.
• LPN 2 works Monday, Tuesday, Friday, and Saturday during Week 1 and Sunday, Tuesday, Wednesday, and Thursday during Week 2.
• Opportunities performed on-site at this work location at least three days a week may be eligible for an additional 5% premium pay in addition to the advertised salary.
• King County employees receive 5% premium pay in addition to the advertised salary and are eligible to receive a free ORCA card for transportation use.

About Lake Burien Transitional Care Facility

The Lake Burien Transitional Care Facility (LBTCF) is a newly developed therapeutic program operated by the Washington State Department of Social and Health Services, in partnership with the Department of Children, Youth and Families and the Health Care Authority. Located in a staff-secure, youth-focused facility in Burien, LBTCF serves youth ages 13 to 17 with intellectual and developmental disabilities, autism spectrum disorder, and co-occurring behavioral health needs. The program provides whole-person, trauma-informed care using adapted evidence-based therapies and a person-centered approach that emphasizes strength-building, skill development, and family engagement. With space for up to 12 participants, LBTCF is designed to stabilize youth in crisis and support their successful transition to a home or community-based setting.
