City/State: Nyack, New York Department: Cancer Center - Administration Work Shift: Day Work Days: MON-FRI Scheduled Hours: 8 AM-4:30 PM Scheduled Daily Hours: 8 HOURS Hours Per Pay Period: 40 Pay Rate/Range: 0.00 Job Summary Assists breast surgeon; participates in the assessment and treatment of breast surgery patients as authorized by state and local health codes, the Montefiore Nyack Hospital (MNH) Medical Board, and the supervising physician. Responsible for coordinating the interdisciplinary team of providers to best serve the newly diagnosed breast cancer patient, including assessment, documentation, education, and continuity of care for the patient and family. Identify and assist patients and families in eliminating potential barriers to care by identifying and referring to appropriate resources and serve as a clinical resource with expertise in oncology care management. In collaboration with the Breast Care Team and Cancer Committee, coordinates patient care services and outreach components of the program. This includes monitoring patients through screening programs and following patients diagnosed through their cancer treatment planning and survivorship. Assists in development and provision of education, prevention, early detection programs, referring high risk patients to appropriate genetic counseling and high risk program along with other activities as requested for compliance with NAPBC and CoC standards. Coordinate and schedule surgical procedures, follow ups with integrated health team members. Working closely with Tumor Registry staff as part of the Integral member of the multidisciplinary team of care providers who reflect a culture of cooperation, enthusiasm and mutual respect. Navigating patients through the healthcare system, i.e. providers' offices, hospitals, insurance and payment systems, patient-support organizations and other components of the health care system. In addition, the Patient Navigator helps to identify barriers preventing patients from receiving timely and quality healthcare, working to remove those barriers. Essential Functions Demonstrates the knowledge and skills necessary to assess and meet the physical, psychosocial, educational and safety needs of breast patients and their families. Performs and/or provides 1st assist during surgical breast procedures, for which she has demonstrated professional competence, and within the scope of practice of the supervising physician; provides post-operative management and follow-up care of patients undergoing surgical breast procedures. Coordinate and schedule surgical procedures. Obtain authorization if required. Performs and/or orders routine lab tests and diagnostic procedures as approved by the MNH Medical Board, Director of service and supervising physician. Reviews all findings with supervising physician and records them in the medical record; keeps complete and organized records of all events and patient encounters Orders and/or administers medication and obtains all signatures and authorizations of responsible physician for all medications prescribed and/or dispensed. Works collaboratively with physicians, nurses and other healthcare members in planning for patient care and in the routine management of assigned patients, making referrals to other disciplines as indicated Facilitates access to available resources for obtaining insurance coverage and/or sliding fee benefits. Identifies and maintains materials and resources for breast cancer patients. In collaboration with other members of the healthcare team, build partnerships with local agencies and groups that may assist with cancer patient care, support, or educational needs. Assist with completing a treatment summary and survivorship care plan for all eligible cancer patients at the completion of Cancer Treatment as outlined in the standards of the ACOS, CoC and NAPBC. Track, monitor and evaluate with administration of survivorship care plans monthly. Report quarterly to Cancer Committee. Assist in development and provision of education, prevention, early detection programs, and other activities as requested for compliance with CoC and NAPBC standards Review the COC needs assessment to address health care disparities and barrier to cancer care. Based on COC recommendations identify barriers and provide resources to patients. Track referrals. Document monitoring, evaluation, and findings of patient navigation process including health disparity populations served and barrier(s) that are addressed. Report quarterly to Cancer Committee. Fields physician related clinical calls and handles appropriately. As required, creates and maintains a variety of databases for clinical research, patient tracking purposes or grant applications. Serves on Cancer Committee and actively participates in meeting standards for accreditation. Assists in gathering patient cancer cases and participates in multidisciplinary breast cancer conferences/tumor boards. Participates in the department’s performance improvement activities Participates in community outreach and educational programs and committee work as assigned. Complies with licensure and/or certification requirements. 4. Participates in annual competency assessment. BUDGET 1. Demonstrates fiscally prudent use of equipment and supplies. 2. Qualifications BSN Required Experience as a navigator and patient educator in oncology.
