Care Coordination Case Manager

A healthcare organization in Brooklyn seeks an experienced Case Manager to assess and implement healthcare plans. Responsibilities include working collaboratively with clients, developing tailored plans, and continually evaluating client care needs. Candidates should hold a Master's in Social Work or an R.N. with a Master's in a health-related field and have at least two years of relevant experience.
