Role Description This is an Independent contractor, at patient's home Physical Therapist role based in Miami, FL. The Physical Therapist will evaluate patients, develop individualized treatment plans, and provide evidence-based interventions to improve mobility, strength, and functional independence. Daily tasks include conducting thorough assessments, delivering manual therapy and therapeutic exercises, educating patients and caregivers on home programs and safety, and documenting all care in accordance with regulatory and organizational standards. The role involves close collaboration with physicians, nurses, and other rehabilitation professionals to coordinate patient care. The Physical Therapist will also participate in case conferences, monitor progress, adjust treatment plans as needed, and support smooth transitions to home care or other settings. Qualifications Candidates should possess strong Physical Therapy and Rehabilitation skills, with the ability to design and implement individualized treatment plans. Candidates should possess solid Patient Care and Home Care skills, including patient and caregiver education and safe discharge planning. Candidates should possess foundational knowledge of Medicine relevant to musculoskeletal, neurological, and cardiopulmonary conditions. Active state licensure as a Physical Therapist in Florida (or eligibility to obtain), and graduation from an accredited Physical Therapy program. Experience in outpatient, acute, or home-based rehabilitation settings is beneficial. Strong clinical reasoning, documentation, and time-management skills, with the ability to work effectively in an on-site, interdisciplinary environment.
