Job Summary:The Clinical Care Coordinator supports value-based care initiatives by assisting with care management, chronic disease monitoring, and coordination of services across the healthcare continuum. This role focuses on improving patient outcomes, reducing care gaps, and supporting quality performance through proactive patient engagement and collaboration with interdisciplinary teams.

Preferred Qualifications

• Experience with Chronic Care Management (CCM) programs 
• Experience in FQHCs, primary care, or population health settings 
• Familiarity with value-based care models and quality metrics

Reports to: Director of Healthcare Operations

Position Purpose
The Senior Behavioral Health Manager provides clinical, operational, administrative, and strategic leadership for Behavioral Health services across Family Medical Centers (FMC). The position leads and develops Behavioral Health staff, ensures effective and compliant service delivery, advances integrated care, monitors performance and access, and supports the quality, sustainability, and growth of Behavioral Health programs.

The Manager serves as a working clinical leader , balancing direct patient care with staff supervision, program management, operational oversight, quality improvement, and organizational collaboration.

1. Clinical Leadership & Practice

Key Responsibilities

• Provide clinical leadership, consultation, and support to Behavioral Health clinicians within the scope of professional licensure. 
• Provide direct behavioral health services, including assessment, diagnosis, treatment planning, intervention, care coordination, and referral. 
• Maintain an appropriate clinical caseload while balancing leadership and administrative responsibilities. 
• Promote evidence-based, trauma-informed, culturally responsive, and patient-centered care. 
• Support consistent clinical standards, workflows, protocols, and treatment practices across FMC locations. 
• Participate in case consultation, multidisciplinary meetings, care planning, and clinical initiatives. 
• Monitor clinical practices and identify opportunities to improve patient outcomes and continuity of care.

2. Behavioral Health Integration & Interdisciplinary Collaboration

Key Responsibilities

• Strengthen integration of Behavioral Health with primary care through team-based care, warm handoffs, screenings, consultation, and care coordination. 
• Develop collaborative workflows between Behavioral Health clinicians, PMHNPs, primary care providers, and other care team members. 
• Facilitate communication and consultation between therapy and psychiatric medication management. 
• Promote continuity of care and reduce fragmentation between behavioral health services. 
• Partner with the Medical Director and PMHNPs to identify clinical and operational gaps. 
• Participate in interdisciplinary case consultation and care planning. 
• Establish clear communication pathways among Behavioral Health, primary care, psychiatric services, and support services. 
• Promote a unified Behavioral Health team focused on shared patient care, quality, access, and organizational goals.

3. People Leadership, Supervision & Workforce Development

Key Responsibilities

• Provide administrative and clinical supervision, coaching, mentoring, and performance management for Behavioral Health staff. 
• Establish clear expectations related to productivity, access, documentation, quality, compliance, teamwork, and patient care. 
• Conduct regular performance feedback and development activities. 
• Identify training, professional development, and workforce needs. 
• Participate in recruitment, interviewing, onboarding, retention, succession planning, and workforce planning. 
• Address performance, attendance, conduct, and operational concerns in collaboration with Human Resources and organizational leadership. 
• Monitor staffing levels, workloads, coverage, and service capacity. 
• Ensure staff maintain appropriate licensure, credentialing, privileging, certifications, and continuing education.

4. Behavioral Health Operations & Access Management

Key Responsibilities

• Provide day-to-day operational leadership for Behavioral Health services. 
• Monitor appointment availability, access, wait times, no-shows, patient volume, provider capacity, and productivity. 
• Establish and monitor operational and productivity expectations in collaboration with organizational leadership. 
• Evaluate staffing and service delivery needs based on demand, clinical need, access, productivity, and resources. 
• Conduct regular operational rounding across FMC locations. 
• Identify workflow inefficiencies and implement process improvements. 
• Maintain appropriate staffing coverage and continuity of care during vacancies, absences, and periods of increased demand. 
• Monitor operational performance and communicate trends, barriers, risks, and recommendations to the Director of Healthcare Operations. 
• Support implementation and evaluation of operational changes affecting Behavioral Health.&n
