Complete OASIS assessments and individualized plans of care. Deliver skilled nursing care and patient education. Coordinate care with physicians and interdisciplinary team members. What We\'re Looking For Active Minnesota Registered Nurse (RN) license. 3+ years of RN experience (home health experience preferred). Strong clinical assessment and critical thinking skills.
