Job Description:\n\nTitle: Direct Support Professional Department: Residential/Day Reports: Residence Manager Location: Residential Services, NY FLSA Classification: Non-Exempt Salary: $18.00 per hour. POSITION SUMMARY: The Direct Support Professional is responsible to provide hands on direct care services for the individuals the agency serves according to state and federal regulations, agency policy and procedure and according to the agency’s mission. The primary role of the Direct Support Professional (DSP) is to work alongside the people we support in order to help them to achieve their personal valued outcomes, be in safe and positive environments and live quality lives by their choices. This includes working with them to develop daily living, professional and informed decision-making skills while also supporting their health and safety. The Direct Support Professional is responsible for the health, safety, and wellbeing of the individuals either living in a residential program, in their own home, attending day program or at a site-based program. The Direct Support Professional provides support in a person driven program in which people with disabilities have choices, control, and access to an array of quality services that assure optimal outcomes such as independence, health, community integration and inclusion, individuality, productive along with a quality of life. The Direct Support Professional provides care, supervision, support, and guidance of all assigned individuals to ensure a safe, healthy, secure and pleasant environment, which promotes and reinforces individual growth and development towards greater independence in self-care, daily living skills, socialization and community living. The Direct Support Professional provides direct care to individuals with intellectual and/or developmental disabilities in residential, site-based settings and/or community; assists and inspires individuals to tend to their personal daily living needs and develop the vocational, nutrition, social and personal skills necessary to achieve the highest possible level of independence in the least-restrictive environment. The Direct Support Professional promote productivity, inclusion, independent and individuality. The tasks and activities performed by a Direct Support Professional depend on the abilities and needs of the individuals being served and the setting to which a position is assigned. ESSENTIAL RESPONSIBILITIES: Provide the day-to-day services in accordance with the organization’s mission, vision, and values; in addition to provide services according to agency policy, OPWDD, OMIG, HIPAA and other state oversight guidelines according to training provided. Uses positive and respectful verbal, non-verbal and written communication a way that can be understood by the individual, and actively listens and responds to him or her in a respectful, caring manner.Prepares and submits all required reports, incident reports in accordance with agency and departmental Quality Assurance, Corporate Compliance and HIPAA policies and procedures. Complete reports and ensure proper documentation of any incident at the facility.Observes and documents individuals' behavior and responses to programs, support services, and treatments and assists in the development, implementation, and evaluation of service and support plans based on individuals' preferences, needs, and interests. Participates in treatment team meetings, communicates and discusses observations, and may suggest alternative programs and services based on individuals' personal preferences.Helps individuals lead richer lives, participate in the community and recreation programs that strengthen life skills, and develop and maintain positive relationships with peers, family members, and others in the community.Follow all plans included but not limited to; behavior plans, nursing plans, habilitation plans, facility plans, etc.Encourages individuals to participate in social, recreational, and leisure activities; and organizes, participates in, and/or supervises such activities.Helps individuals to be safe and learn to be safe in the community and transports and/or arranges for the transportation of individuals to social events and accompanies individuals to community activities.Supports individuals' health and wellness, and safety in everyday situations, assists and/or instructs individuals in performing personal care and hygiene tasks, such as bathing, toileting, grooming, dressing, dental care, nutrition, and daily living activities and teaches individuals basic food preparation, safety, and sanitation skills; supervises and assists in meal planning, food selection, and preparation activities; and feeds or assists in feedings individuals who require assistance.Administers the taking of medication in accordance with the regulations and policies and procedures of the agency governing medication administration. Administer medications and basic treatments as prescribed; observe and monitor individuals in the self-administration of medication; and take and record individuals' temperatures, blood pressures, pulse rates, and weight. Gives medications or assists the individual to take medication while following all laws and safety rules.Accompanies individuals to medical, dental, and health-related appointments or emergencies. Responds to and assists in crisis and emergency situations following established guidelines, procedures, and first aid techniques.Maintains and ensures a safe, clean, and comfortable environment for individuals while assisting individuals in routine housekeeping, cleaning, and minor maintenance tasks.Works in partnership with the individual to track progress toward goals and adjust services as needed and desired by individual, gathers and reviews information about an individual to provide quality services, completes and submits documentation of services on time.Follows the Life Plan protocols and helps with the assessment process by gathering information from many sources. Uses the results of assessments to discuss options with individual and team members to guide support work and collects data about individual goals and satisfaction with services.Recognizes risk and works to prevent an individual’s crisis in a way that meets the individual’s need and uses positive behavior supports to prevent crisis and promote health and safety and appropriate and approved intervention approaches to resolve a crisis and follows Behavior Crisis Protocols.Seeks help from other staff or services when needed during a crisis and own potential role within a conflict or crisis and changes behavior to minimize conflict. Supports the emotional, and social wellbeing of the individual.Monitors for possible signs and symptoms of illness and follows Nursing Protocols and assists the individual to learn disease prevention and maintain good health. Assists the individual to use infection control procedures and prevent illness and helps the individual make and keep regular health and dental care appointments and follow health care plans and use medical equipment as needed. Helps the individual to learn the signs of common health problems and take actions to improve health.Demonstrates the ability to identify, prevent, and report situations of abuse, exploitation, and neglect according to laws and agency policies, recognizes signs of abuse and neglect, including the inappropriate restraint use, and works to prevent them and reports all real or suspected abuse to authorities as mandated by Justice Center and agency procedures. Prevent, recognize and report abuse toward the people we support.Maintains the safety of an individual in the case of an emergency and follows universal precautions and gives first aid as needed in an emergency.Follows relevant laws, regulations and is guided by ethical standards when doing work tasks. Supports individual in a collaborative manner and maintains professional boundaries.Demonstrates professionalism by being on time, dressing appropriately, and being responsible in all work tasks while respects the individual and families’ rights, respect and dignity.Fosters team building and positive productive work relationships on site and ensures the implementation of the Direct Support Professional OPWDD Core Competencies. Ensure implementation of the Direct Support Professional Core Competencies and elements are delivered to individuals.Assists the individual to set goals, make informed choices, and follow-through on responsibilities, support self-advocacy and advocate for individuals when needed to pursue needed services, support and resources.Responsible to ensure that complete, accurate, legible documentation is done contemporaneously and in accordance with policies for all program systems such as but not limited to: fire drills, daily services, nursing protocols, behavior data, triage and crisis calls, overnight data, community inclusion, personal spending, etc.Ensuring goals are met in areas including client and family satisfaction, safety, quality, and team member performance. Provides person centered support and helps the individual to build on his or her strengths in life activities.Assists the individual meet his or her physical and personal care needs (i.e. toileting, bathing, grooming) and provides training in these areas when needed. Teaches and assists the individual with household tasks such as laundry and cleaning and assists the individual to learn about meal planning and shopping, and safe food preparation.Ensures that all necessary food and other household items are in ample supply and are stored properly and ensure that an individual’s diet is followed, and food and drink are served according to their prescribed dietBe sure the facility remains clean and sanitary. Make report concerns observed for physical plant of the facility, including but not limited to fire extinguisher/fire safety/smoke alarm checks, seasonal equipment checks (air conditioners, boilers, screens etc.), broken furniture, hazards, cleanliness; and noting any needed materials or items.Follow the Staff Assignments and Programs Structured SchedulesTransports individuals as necessary to programs, activities, appointments, recreational functions, etc. as scheduled or requested by supervisors. Drive program vehicles in safe and responsible manner and document all vehicle operations in travel logs.Ensure adequate supervision and that all safeguards and plans are followed; in addition to monitor the interaction, activities and response to services. Ensure to follow mealtime safeguards to include but not limited to; prescribed diets, meals plan and choking preventions.Utilizes effective professional and empathetic interpersonal skills when counseling, supporting or negotiating to handle sensitive issues with families and outside agencies. Work with the people we support to help them identify and realize their personal life goals and effectively communicate and support the organization’s mission, vision and valuesAdvocate with the people we support in a variety of situations and work with the people we support to teach them new life skills and work with the people we support and other members of the interdisciplinary team to develop service plans that aid the person in achieving their personal life goals.Accurately document service provision, daily notes and other information as required and provide positive behavior support to the people we support as necessary. Work in partnership with the people we support to promote their health and safety.Adhere to the organizations Code of Ethics and communicate in a respectful manner with all people including the people we support, supervisors, other valued team members, and members of the community. Respects the cultural needs and preferences of each individual and co-workers.Encourages and assists individuals in connecting with others and developing social and valued social and/or work roles based on his or her choices. Supports individuals to connect with friends and to live and be included in the community of his or her choice, helps individuals transition between services, adapt to life changes, i.e., moving into home & community-based settings and respects the role of family members in planning and providing services and plans and initiates a variety of social and recreational activities both in the home and in the community for the individuals and supports individuals in their involvement in these activities.Completes required ongoing training and continues to develop skill and any additional trainings as needed/required by program manager to meet an individual’s needs.Understands the purpose of adaptive equipment; makes recommendations as to adaptive equipment that would allow the individual to be more independent and functional; ensures adaptive equipment is available and used by the individual.Manage all individual’s funds according to policy & procedure and state regulations and effectively manages petty cash, receipts, and documentation by the end of every shift worked.Communicates relevant information at change of shift and to supervisor.Performs other duties and responsibilities as assigned. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES: Employee must possess the ability to interact with individuals and staff professionally and ethically, understand and implement service plans, and to complete all required documentation clearly, legibly in a timely manner. Must have a passion for and desire to work with people with intellectual disabilities and other physical or mental disabilities. Knowledge of basic human physical and psychological needs. Knowledge of simple health and safety precautions. Ability to complete data collections and follow safeguard and program plans. Can communicate orally and in writing in English, bilingual is a plus. Able to write summary reports using a professional, objective style and voice. Ability to use computers and stay current with technology used for documentation (EVV, HER, EMR), communication, and other work activities. Ability to interact with individuals, community members and other treatment team members. Ability to read and comprehend technical training manuals. Ability to follow written and oral instructions course of action. Ability to learn basic operation and maintenance of adaptive equipment. Ability to be supportive and patient. Ability to be a team player. Ability to observe, record and report participant’s behavior, attitude, and physical condition. Ability to be flexible in the event of a staffing emergency.
